Transferring Prescriptions

We're thrilled you've decided to transfer your medications to a UnityPoint Health pharmacy. 

Step 1: Gather up your information:

• Rx Number(s)
• Drug Name(s)
• Physician's name and phone number
• Current pharmacy name and phone number

Step 2: Do one of the following:

• Call the UnityPoint Health pharmacy nearest you and provide them with your medication information.
• Complete the Combined Release Authorization and Transfer Form for the location near you and follow the submission instructions.

Please note: Prescriptions cannot be transferred from mail order pharmacies. New prescriptions will need to be obtained.
