Ordering a New Prescription

Ordering your medications is simple.

Step 1: Have your doctor electronically send your prescription to the UnityPoint Health pharmacy nearest you.

Step 2: Patient Information Form can be completed prior to any prescriptions being filled. Select the form for your location.
